It's not always a clean split. Some orders straddle the line, like a standard consumable with a non-standard shipping requirement. In that case, treat it like Scenario B until you have written confirmation on the shipping terms. Approving a rush fee immediately made me think, "could I have negotiated?" I didn't relax until the delivery arrived on time and correct.
